****ลดการก่อตัวของแผลเป็นในระหว่างกระบวนการสมานแผล reducing scar formation during wound healing process โดยสามารถช่วยลดการก่อตัวของแผลเป็นโดยกระตุ้นการเปลี่ยนแปลงในอัตราส่วนของ Collagen type III และ I เพิ่มการควบคุม MMP-1 และ MMP-9 และลด TiMP2 ด้วยกลไกเหล่านี้ exosomes ช่วยส่งเสริมการปิดของแผลและลดการเกิดแผลเป็นได้

•ช่วยในการปลูกถ่ายไขมัน fat grafting จากการเพิ่มหลอดเลือดไปเลี้ยงเซลล์ increases angiogenesis •ช่วยการหายของแผลของผู้ป่วยเบาหวาน โดยช่วยเพิ่มหลอดเลือดการเพิ่มการสร้างคอลลาเจนของเซลล์ไฟโบบาส

•และทำหน้าที่เป็นพาหะและโครงร่างรวมสำหรับการรักษา

•ลดการเกิดแผลเป็น โดยกระบวนการซ่อมแซมผิวหนังแบบไร้แผลเป็น Scarless cutaneous repair โดยพบว่าสามารถลดขนาดของแผลเป็น เพิ่มสัดส่วนของ Collagen type III: I , เพิ่มการแสดงออกของเซลล์ไฟโบบาสเป็นต้น

แต่อย่างไรก็ตามการใช้ Exosome ในการรักษาหลุมสิวรวมถึงแผลเป็นต่างๆ ยังมีข้อจำกัดเนื่องจากยังมีการศึกษาไม่มากและมีความหลากหลายของการได้มาของ Exosome ด้วยครับดังนั้นต้องรอการศึกษาเพิ่มเติมต่อไปครับ

Skin wound healing is an intractable problem that represents an urgent clinical need. To solve this problem, a large number of studies have focused on the use of exosomes (EXOs) derived from adipose-derived stem cells (ADSCs). This review describes the mechanisms whereby ADSCs-EXOs regulate wound healing and their clinical application. In the wound, ADSCs-EXOs modulate immune responses and inflammation. They also promote angiogenesis, accelerate proliferation and re-epithelization of skin cells, and regulate collagen remodelling which inhibits scar hyperplasia. Compared with ADSCs therapeutics, ADSCs-EXOs have highly stability and are easily stored. Additionally, they are not rejected by the immune system and have a homing effect and their dosage can be easily controlled. ADSCs-EXOs can improve fat grafting and promote wound healing in patients with diabetes mellitus. They can also act as a carrier and combined scaffold for treatment, leading to scarless cutaneous repair. Overall, ADSCs-EXOs have the potential to be used in the clinic to promote wound healing.

ADSCs and ADSC‐Exos Indirectly

Regulate Fibroblasts and Myofibroblasts by Promoting Angiogenesis and Inhibiting Inflammation, Thereby Reducing Scar Formation

ADSC-Exos can effectively inhibit the proliferation and migration of hypertrophic scar fibroblasts, reduce the expres- sion of Col-1,Co-III, α-SMA, IL-17RA and p-Smad2/p- Smad3, and increase the level of SIP1 in fibroblasts. Mice treated with ADSC-Exos showed faster wound healing and less collagen deposition miR-192–5p is also the regula- tory mode of ADSCs to reduce the fibrosis level of hyper- trophic scar. The effects of ADSCs on the biological characteristics of fibroblasts and myofibroblasts during the process from wound to scar.
