แต้ม TCA CROSS รักษาหลุมสิว รอยแดงและรอยดำ บอกอะไรเราได้บ้างครับ?

หลุมสิวแบบจิกลึก หรือ ice-pick acne scar มีลักษณะเป็นหลุมแคบ ลึก และมักตอบสนองต่อการรักษาแบบผิวเผินได้จำกัดครับ หนึ่งในวิธีที่แพทย์อาจพิจารณาคือ TCA CROSS หรือการแต้มกรดไตรคลอโรอะซิติก (trichloroacetic acid; TCA) ลงเฉพาะในก้นหลุมสิว

หลักการของ TCA CROSS คือ ทำให้เกิดการซ่อมแซมผิวแบบควบคุมเฉพาะบริเวณหลุมสิว เพื่อกระตุ้นการปรับโครงสร้างคอลลาเจนในระยะต่อมาครับ จุดเด่นคือสามารถลงการรักษาแบบเฉพาะตำแหน่ง จึงเหมาะสำหรับหลุมแคบและลึกมากกว่าแผลเป็นชนิดกว้างหรือมีพังผืดยึดรั้ง [1,2]

อย่างไรก็ตาม การรักษาหลุมสิวไม่มีวิธีใดเหมาะกับทุกชนิดครับ หลุมชนิด rolling scar อาจต้องประเมินการตัดพังผืดร่วมด้วย ส่วนหลุม boxcar ที่กว้างอาจต้องวางแผนร่วมกับหัตถการอื่นตามลักษณะและความลึกของแผลเป็นแต่ละคน

หลังแต้ม TCA อาจเกิดสะเก็ด รอยแดง หรือรอยคล้ำชั่วคราวได้ครับ

* รอยแดงและเส้นเลือดฝอยที่ขยาย สะท้อนว่าผิวยังมีการอักเสบและอยู่ในช่วงฟื้นตัว
* รอยดำ หรือ PIH เกิดจากการสร้างเม็ดสีเพิ่มขึ้นหลังการอักเสบหรือระคายเคือง

หลังการแต้มกรด TCA เพื่อรักษาหลุมสิว บริเวณที่แต้มอาจมีสะเก็ด รอยแดง หรือรอยคล้ำตามมาได้ครับ เนื่องจากผิวกำลังเข้าสู่กระบวนการอักเสบและซ่อมแซมตัวเองอย่างเป็นลำดับ

เมื่อดูด้วยตาเปล่า เราอาจเห็นเป็นรอยสีแดง–น้ำตาลหลายจุด แต่การตรวจด้วยเดอร์โมสโคปช่วยให้เห็นรายละเอียดของผิวได้มากขึ้นครับ

รอยแดงและเส้นเลือดที่ขยาย
ลักษณะสีชมพู–แดงร่วมกับเส้นเลือดฝอยที่เห็นชัดขึ้น สะท้อนว่าผิวยังมีการอักเสบและการตอบสนองของหลอดเลือดอยู่ครับ ในช่วงแรกอาจเป็นส่วนหนึ่งของการฟื้นตัว แต่หากรอยแดงอยู่นานขึ้นหรือมีอาการแสบ คัน หรือบวมร่วมด้วย ควรให้แพทย์ประเมินครับ

รอยดำหลังการอักเสบ
ส่วนรอยสีน้ำตาลหรือคล้ำ เกิดจากการสร้างเม็ดสีเมลานินเพิ่มขึ้นหลังผิวมีการอักเสบหรือระคายเคือง เรียกว่า post-inflammatory hyperpigmentation หรือ PIH ครับ พบได้มากขึ้นในผู้ที่มีผิวคล้ำง่าย หรือมีประวัติเป็นรอยดำหลังสิวและหัตถการ [1,2]

ดังนั้น หลังทำ TCA CROSS การดูแลผิวไม่ใช่เพียงรอให้สะเก็ดหลุดครับ แต่มีส่วนสำคัญต่อโอกาสเกิดรอยแดงและรอยดำในระยะยาวด้วย

* ไม่แกะ เกา หรือขัดถูสะเก็ด
* ใช้คลีนเซอร์และมอยส์เจอไรเซอร์ที่อ่อนโยน
* งดสครับ กรดผลัดเซลล์ผิว และเรตินอยด์ขณะผิวยังระคายเคือง
* ป้องกันแดดสม่ำเสมอด้วยครีมกันแดดชนิดปกป้อง UVA/UVB เพราะแสงแดดทำให้รอยดำเข้มขึ้นและจางช้าลงได้ครับ [2,3]

หากมีอาการปวดมาก บวมแดงลาม มีตุ่มน้ำ หนอง หรือแผลไม่ดีขึ้น ควรกลับมาพบแพทย์ครับ

TCA CROSS creates a controlled healing response within the scar, supporting collagen remodeling over time. Its targeted approach makes it particularly relevant for narrow, deep ice-pick scars. In contrast, rolling scars may require assessment for tethering and possible subcision, while broader boxcar scars may need a different or combination treatment plan. [1,2]

After TCA CROSS, small crusts, temporary redness, and dark marks can occur as the skin heals.

Dermoscopy can help document these clinical features during follow-up. Persistent redness, worsening dark marks, burning, itch, or swelling should be assessed by a physician.

Aftercare is an important part of treatment:

• Do not pick, scratch, or exfoliate crusts.
• Use a gentle cleanser and a non-irritating moisturizer.
• Avoid scrubs, exfoliating acids, and retinoids while the skin remains irritated.
• Use broad-spectrum UVA/UVB sunscreen consistently, as sun exposure can worsen and prolong dark marks. [2,3]
