ส่วนมากสิวที่หลังและลำตัวมักจะตอบสนองช้ากว่าการรักษาสิวที่ใบหน้าแต่มีการศึกษาวิจัยเกี่ยวกับการรักษาสิวที่หลังลำตัวน้อยกว่าที่ใบหน้า
เช่น
•การผลัดเซลล์ผิว เช่น salicylic acid, glycolic acid, lactic acid, mandelic acid, retinoic acid, trichloroacetic acid, Jessner’s solution, kojic acid, pyruvic acid, azelaic acid และการใช้ combination peel ที่มีการรวมของกรดหลายชนิด
•เทคนิค Photodynamic therapy (PDT) มีการศึกษาในการใช้ดูแลรักษาสิวที่หลังลำตัวค่อนข้างมาก สามารถช่วยลดสิวอักเสบได้ โดย เช่นการใช้ topical 5% 5-aminolevulinic acid (ALA) (แนะนำ)
•กรณีเป็นแผลเป็นจากสิวที่ลำตัว เช่น การฉีดยา corticosteroids, 5-fluorouracil, bleomycin ร่วมกับหัตถการ cryotherapy and PDT
•การรักษาด้วยแสงเลเซอร์ความเข้มข้นต่ำ Low level laser therapy LLLT (Healite) (แนะนำ)

7.เทคนิครักษาแผลเป็นจากสิวที่หลังและลำตัว
•โดยสิวที่หลังและลำตัวมักจะเกิดเป็นแผลเป็นรุนแรง ทั้งเป็นยุบตัว Atrophic scar และแผลเป็นนูนคีลอยด์ Keloid

•การรักษารอยดำรอยแดงรวมถึงแผลเป็นเช่นการใช้เลเซอร์ IPL, QsNdYag Laser, Diode Laser, Long pulsed NdYAG เป็นต้น

7.1แผลเป็นนูนคีลอยด์ Keloid มักเป็นมากที่ไหล่และหน้าอกส่วนบนครับ
แผลเป็นนูน Hypertrophic Scar และคีลอยด์ Keloid ใช้การฉีดยาสเตียรอยด์ Intralesional Steroid Injections
+ การใช้เลเซอร์เช่น Pulsed Dye laser PDL (Vbeam), Fractional Non Ablative Laser, Fractional CO2 Laser, Fractional Erbium YAG Laser
+ จี้เย็นด้วยไนโตรเจนเหลว Liquid Nitrogen Cryotherapy

7.2แผลเป็นยุบตัว Atrophic acne scar
การรักษาใช้เลเซอร์เช่น Fractional Ablative Laser, Fractional CO2 Laser, Fractional Erbium YAG Laser, InfiniRF Microneedle, Ematrix, Picosecond Laser เป็นต้นครับ

7.3 แผลเป็นจากสิวที่ค่อนข้างเป็นเฉพาะสำหรับสิวที่หลังลำตัวและแขนส่วนบน คือ follicular macular atrophy หรือ Papular acne scar มักไม่ค่อยตอบสนองต่อการทายารักษา มีลักษณะเป็นตุ่มนูนค่อนข้างกลม ขนาด 1-4 มม. มีสีขาว สีเหลือง หรือเป็นลักษณะสีผิวครับ
การรักษาเช่นใช้เลเซอร์ Fractional Laser ,Ablative Laser, Picosecond Laser
•การใช้อุปกรณ์คลื่นวิทยุ Radiofrequency RF เช่น InfiniRF Microneedle, Ematrix
•Needle-Guided Radiofrequency Ablation*

จะเห็นว่าการเป็นสิวที่หลังและลำตัวมีโอกาสเกิดแผลเป็นชนิดต่างๆตามมาได้ดังนั้นการดูแลรักษาจึงมีความสำคัญ

Truncal acne: acne vulgaris involving the chest, back, or shoulders , is thought to be present in more than half of patients with acne. Because the study of acne has been focused on the face, there are sparse data in the literature about the clinical characteristics and management options for truncal acne. It is thought that the pathogenic process is similar between the face and the back, suggesting that treatments studied on the face may be suitable for use on the back. It is not uncommon for patients to omit discussion of truncal acne with their treating physician, and it is likely that non-facial acne is under-diagnosed and under-treated. Scarring and pigmentary problems are common sequelae of acne on the trunk, underscoring the need for early and effective treatment as a preventive measure. Truncal acne merits consideration and should be more thoroughly studied. Initiating treatment with a topical retinoid combined with an antimicrobial agent is logical for most patients with truncal acne, and mirrors recommendations for facial acne; however, there are additional considerations for truncal acne such as extension of the lesions, risk of antibiotic resistance due to large surface areas treated for a prolonged duration, accessibility of lesions, and discoloration of clothing or bedding due to topical products. Oral isotretinoin is the treatment of choice when truncal acne is severe.
